Red alerts: Safety Incidents/unsafe conditions with risk of serious consequences
Green alerts: Incidents/unsafe conditions with minor consequences

Describe:
In subject frame:
"Safety Incident"
In text frame:
- Rig Name
- Description of incident
- Equipment involved
- Technical cause (detailed description of parts involved etc.)
- Consequences
- Possible consequences for other users of the product
- If an internal report is issued, please attach this to the mail
- You can also use the attached incident form
When issuing an incident report to Aker MH, please note that involved equipment parts etc. must be kept for later cause analysis.
